Summer Registration 2021 -                        Private Lessons and Classes
Welcome! This form is a complete registration form for all classes and lessons for the Azusa Conservatory of Music. Please be prepared to register for all classes and private lessons that you plan to participate in! Full class descriptions are listed along with prices. This form needs to be filled out for each child/person you are registering.

Please note the summer 2021 sessions will be fully on Zoom. In person teaching will start in September. You will get a zoom link after you register.

Summer Semester: 8 Saturdays, June 12th, 2021 - July 31st, 2021**

**SHINE CHOIR: Thursdays, 11:00am - 12:00pm,  Summer Semester 8 Thursdays, June 10th - July 29.
**Note that there may be some flexibility for exact time and day for summer classes and private lessons. We try to plan the majority of our lessons on Saturday mornings, but please contact us if you have scheduling requests or questions.
**Lessons and Classes: June 12th - July 31st time/date subject to adjustment

*For all minors (under 18), legal parent or guardian must be present in person on the first zoom call lesson to confirm and approve the lessons or make a separate zoom appointment to verify and approve the lessons.

Summer Semester 8 lessons June 10-July 31, 2021
Private lessons are 30 minutes per Saturday (some weekday lessons are available) and consists of 8 weeks. Payment must be made in full either before or by the first day of the semester, unless a payment plan has been established. Prices are listed in this registration form.

The conservatory holds classes primarily on Saturdays. Saturday lessons and classes take place between 8:30 AM and 1:30 PM, and take place online via Zoom. Because we are moving into the Summer Semester, there is a little more room to negotiate lesson times according to both teacher and student availability.
