Please note: It is okay if your n95 or surgical-style masks are opened/unsealed as long as they have not been used. Please wash your hands with soap for 20 seconds and/or use disposable gloves to place your unsealed items in a clean Ziploc-style bag. Put a note in the bag that includes: (1) a statement verifying that the masks have not been used, (2) the date the bag was packed/sealed, and (3) any details you have about the make/model and expiration date (if possible).

Because of the volume of donors, we are now asking people who are low risk based on CDC guidance* to drop off their items. We will provide you with the drop off location via e-mail after you complete this form; the drop off does not require any human contact. Alternatively, we pick up donations from individuals who are high risk or who otherwise prefer a pick up.
