The Massachusetts Department of Elementary and Secondary Education (DESE) and Commissioner Riley have mandated public schools return elementary school students to full-time, in-person learning, five days per week, beginning Monday April 5, 2021 and middle school students on Wednesday April 28, 2021. Parents may choose for their child to be “fully in-person” or “fully remote,” the current hybrid model will no longer exist after the date specified above for the applicable grade level.

After review of the information provided in the letter from Superintendent Lang, please complete this form to choose the learning model for your child for the remainder of the current school year.

Please complete and submit a separate form for each child in your family as soon as possible, however not later than Wednesday March 17, 2021.
