Carta CLE Virtual Session Request Form
Please complete this form after watching Carta's MCLE-eligible virtual sessions.  You must complete a separate form for each session viewed if you want a certificate of completion for each session.  All sessions have been approved for MCLE credit in California.    New York attorneys should follow the New York Approved Jurisdiction Policy for the procedure to claim out-of-state CLE credits.

Once this form is submitted, a Carta representative will reach out to provide your certificate of completion for your records.  California attorneys must retain their certificate of completion for at least four years from the date of session completion.
