For RETURNING STUDENTS, you only need to enter mandatory (red star) fields and this year's classes. UNCHANGED DETAILS NEED NOT BE ENTERED. For NEW families and students, please enter all fields with a ** as well.
The email address should be the primary contact email address that will receive all general emails. (The invoice address can be different) This will also be used as the JAL Dance website member's page login username. If there are multiple students within a family, it MUST be the same for each student.
For subsequent students, all that is required is primary email address, student name, birthday, enrolled classes and final declaration.
Enrolment is assumed until the end of 2022 unless JAL Dance is notified via email. Please refer to website for fee schedule and terms and conditions. Notification for cancellation of any lessons must be received prior to the invoicing for the next payment block to avoid being charged for cancelled classes. Participation in the end of year concert is assumed with costumes ordered under that assumption.
