Program Books are discounted to $15 when pre-ordered.

Upon completion of this form, payment for the total number of program books ordered must be submitted to the Distinguished Young Women of Kentucky Treasurer. You have 4 options for submitting payment:

* Credit Card: If paying by credit card, enter the information in the

applicable section below.

* Venmo: @DYWofKY. In the notes area, enter your name, participant's

name, and participant's representative area.

* PayPal: kytreasurer@distinguishedyw.org. In the "What's the payment

for?" section, enter your name, participant's name, and

participant's representative area.

* Mail a check made payable to Distinguished Young Women of KY to:

DYW of Kentucky Treasurer

231 South Stourbridge Rd.

Versailles, KY 40383

In the check's memo section, write the participant's name and

participant's representative area. Enter the Check # where

prompted below.

Parents can pick up their pre-ordered Program Books during the State Week drop-off at KHSAA on January 8th. All other pre-orders will be available for pick-up at the Singletary Center at the designated Program Book table prior to each Preliminary and the Finals showcase.
