Submitting this form means that I have read the Old Forge School Districts Guidelines for the Junior Firefighter Program and understand that these rules were established for my safety, and to protect the Old Forge Volunteer Fire Department and Old Forge School District. I also understand that any breach of these guidelines can result in immediate dismissal from the Junior Firefighter Program, and may result in disciplinary action by school administration, or additional punishment by State and Local Law.
