I, parent or guardian, of the listed child(ren), acknowledge and accept the risk of physical injury associated with participation in Vacation Bible School. Except for gross negligence on the part of the sponsor, I accept personal financial responsibility for any bodily or personal injury sustained during the activity. Further, I promise to hold harmless the sponsoring organization and its representatives for any injury related to the activity. If a dispute over this agreement or any claim for damages arises, I agree to resolve the matter through a mutually acceptable arbitration process.

Refusal to accept the above agreement and release results in the child(ren) not being able to attend Congaree Baptist Church Vacation Bible School

I, parent or guardian, of the listed child(ren), hereby authorize Congaree Baptist Church (CBC) and/or their agents to take photograph(s) and/or videos during VBS. I also understand that these photographs and/or videos will be used in print and/or on the CBC website. I specifically understand that CBC shall hereby retain any and all rights in respect to the photograph(s) and/or video/film project(s) and/or website/internet project(s), including but not limited to, the rights to reproduce, copy/edit, exhibit, publish, or distribute such photograph(s) and/or video/film project(s) and/or website/internet project(s). I further understand that photographs and/or videos may be used in a publication, print-ad, direct mail piece, electronic media, or other form of promotion. I release CBC, CBC employees, CBC designees and the photographer/videographer from liability for any violation of any personal or proprietary right I may have in connection with such use.
