Please fill out the form below to register for the Bethpage School Nutrition Meal Distribution program for delivery service.  Food service director Nick Colasurdo is willing to deliver to the students in the Bethpage school district that are unable to pick up at Bethpage High School.  Pick up is between the hours of 8-10. The delivery service will be held between the hours of 10-12. Please leave the students first and last name, school, home address, phone number, email address and any special information.  All meals will be compliant to the NY state child nutrition program. Social distancing rules will apply. Food will be delivered at the front door unless otherwise noted.  The program will begin on Monday, March 30, 2020 and continue until further notice.  Please fill out a form for each child for households with multiple children.
