Please fill in your preferred appointment dates in the form below, and our team will be in touch to confirm your appointment date and time via WhatsApp. Please do not attend if you do not receive a confirmation.

Kindly note that we are limiting the appointment slots to 2 persons in the same party per slot (4 persons, if you are from the same household. MyKad will be requested for proof), and each person will be required to be fully vaccinated:

2 Doses - 14 days after vaccination date (Pfizer, Astrazeneca, or Sinovac)
1 Dose - 28 days after vaccination date (Johnson & Johnson or Cansino)

Kindly be advised that your temperature will be taken, and our team may request to see your MyKad / Passport and digital vaccination certificate when you arrive for your appointment.

Please note that we reserve the rights to refuse entry if you do not fit the criteria as per the Malaysian Government's guidelines.
