Please complete and submit this Registration Form by Friday 27th October 2023 if you wish your son or daughter to receive the Sacrament of Confirmation in 2024.

Only those already baptised in the Roman Catholic Church are eligible to receive Confirmation.

If your child was baptised in any of the Parish Churches, - (Clondalkin Village, Knockmitten or Clonburris) - you do not need to supply a baptism certificate. The parish will be able to verify your child's baptism in our parish registers.

If your child was baptised in any other Church, you will need to obtain a baptism certificate for your child.
Please contact the Church where your child was baptised and request a baptism certificate.
When you get the baptism certificate, please post it to the Parish Office or email a copy of it to the Parish at sacramentclondalkin@gmail.com 

Proof of baptism is a requirement in order to receive the Sacrament of Confirmation.

The information you provide in this Registration Form will be used by Clondalkin Parish to facilitate the preparation, celebration and registration of the Sacrament of Confirmation. It will not be used for any other purpose.

After Confirmation has taken place, a permanent record of having received Confirmation will be entered in our parish Confirmation register.

A notification of having received Confirmation will also be entered on the child's baptism record. If the baptism took place in another parish a notification will be sent to the Church of Baptism.

The Registration Form will be deleted once Confirmation has taken place and the registration and notification mentioned above has been completed.
