PARTICIPATION LIABILITY RELEASE:On behalf of myself or the minor child named, give permission for named participant to participate in educational activities hosted by Purdue Polytechnic Anderson. I hereby release, waive and forever discharge Purdue Polytechnic, its trustees, directors, officers, employees, agents, partners, and assigns from any and all claims for injuries or damages arising out of the named participant in said activities. This release applies even if the act or omission complained of was caused in whole or in part by the strict liability or negligence in any form of the released party.  I voluntarily accept and assume all risks, known and unknown, and take full responsibility for all activities I hereby willingly undertake.

PHOTO RELEASE: I, the undersigned parent/guardian of the named participant, do hereby consent to the use by Purdue Polytechnic and/or its subsidiaries or partner organizations, of my or my child’s image, voice, or both, in any video, photograph or audio tape, either in whole or in part; regardless of whether these materials are used for fundraising, advertising, publicity or any other legal purpose on behalf of Purdue Polytechnic and its subsidiaries. In addition, I waive any and all claims to further compensation or damages based on the use of my or my child’s image or voice, or both. I also waive any right to inspect or approve the finished photograph or video or audiotape. I understand that this consent is perpetual; that I may not revoke it, and that it is binding on my heirs and assigns.
