Thank you for your interest in the Lamar County School District Pre-K program. You must fully complete this form for your child to be placed on the waiting list. Please read the Pre-K program requirements below and type your name, as the parent/guardian, to indicate your understanding of the guidelines and willingness to comply with all requirements.  
-Child must be  four years old on or before 9/1/24 (not yet 5 years old)
-Child must live in the attendance zone for the school at which he/she is applying
-Parent/Guardian must provide a certified birth certificate, immunization form (MS 121), and two (2) current proofs of residency as outlined on the LCSD website at the time of registration (May 2024).
-Child must attend a screening appointment (between April 22-May 3, 2024).
-Parent must complete all registration steps during Pre-K Registration (between May 13-24, 2024) or the child's spot will be forfeited.*
-Parent must attend Pre-K 101 Parent Meeting on Tuesday, May 21, 2024.
-Parents must provide prompt transportation to and from school.
-Participation is voluntary; however, enrolled students are required to arrive to school on time and follow district attendance policies.
-Parents are required to participated in at least three parent-teacher conferences throughout the year.
-Incomplete applications will not be accepted and late applications will only be considered if space becomes available.

*Note Mississippi Action for Progress Head Start will follow up with families to complete the application process and collect proof of annual household income.
