This form is to collect your personal information for our online church database and our paper directory. You do not need to give all the information requested if you prefer not to.

Church Database:
Information on the database will be available to church leaders and staff. Currently your name will be available online to members of any group you are in - for example, a small group - and if you are a church member your full details (except your age range and anything you've asked not to be included) will be available to other church members, and their information to you. We will let you know which groups we plan to include you in before we do it. You are able to choose which information is made available to others.

We'll send you an email confirming your permissions and how you can access the church database.

Paper Directory:

The paper directory will contain names, email addresses and mobile or landline number for those who have given permission for their details to be published.
