I acknowledge the contagious nature of the Coronavirus/COVID-19 and that fully vaccinated individuals are able to transmit and become infected by the virus. Vaccines are effective at preventing serious outcomes, such as severe illness, hospitalization and death. Vaccination status only changes your risk of getting COVID-19 and becoming sick. It doesn't change your risk of exposure to the virus out in the community.

I further acknowledge that CREATIVE CLAY STUDIO LTD has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that CREATIVE CLAY STUDIO LTD can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, customers, volunteers, monitors, teachers as well as other registered members.

I voluntarily seek a studio space to practice in and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set studio rules and procedures to reduce the spread while attending the studio.

I will not enter CREATIVE CLAY STUDIO LTD unless I can attest that:

* I am not experiencing any symptom of illness such as, cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell (even if I suspect it is due to the COVID-19 Vaccine) nor have I within the last 5 days if fully vaccinated or 10 days if not fully vaccinated.

* I am responsible to know what to do if I am exposed to the virus. I will call Toronto Public health at 416-338-7600 or take the COVID-19 self-assessment tool to determine appropriate steps.  https://covid-19.ontario.ca/self-assessment/

* I will not enter Creative Clay Studio if I've been instructed by the COVID-19 self-assessment tool or by Toronto Public health unit to self isolate.


WAIVER OF LIABILITY & INDEMNIFICATION AGREEMENT

I hereby waive, release and discharge any and all claims and damages for personal injury, death, or property damage which I, may sustain or which may occur as a result of my own participation, with CREATIVE CLAY STUDIO LTD.

I further understand and agree that:

1. This release is intended to discharge in advance CREATIVE CLAY STUDIO LTD, its owners, officers, employees, contractors, directors, agents and successors from and against any and all liability, except for their sole negligence or intentional acts, connected in any way with my participation in CREATIVE CLAY STUDIO LTD activities;

2. Such activities may be of a hazardous, strenuous, and/or physical nature, cause injuries and participation in said activities may result in injury, death or property damage;

3. Knowing the risk involved, nevertheless, I voluntarily request permission for myself to participate in said activities;

4. I hereby assume any and all risks of injury, death or property damage and to release and hold harmless CREATIVE CLAY STUDIO LTD, its officers, employees, contractors & agents, except for their sole negligence or intentional acts;

5. This waiver, release and assumption of risk is binding on me and my heirs and assigns;
