If you are going to miss a rehearsal or performance for any reason this form must be filled out prior to the event you are missing.  Please note...just because you fill out this form does not automatically make it an Excused Absence.  Each absence will be examined by Mr. Bowman.  You will still need to turn in an excuse from doctor, dentist, etc. to Mr. Bowman.
