Canadian thyroid lab testing tips & challenges
This survey publicly monitors the way our health care system treats thyroid patients. We are crowdsourcing data that we can use to argue against health inequities across the country. Data may suggest when harm may be done by lack of testing and when patients are being charged user fees for tests.

This is an anonymous survey.  However, what you enter and write will NOT be confidential. Your responses will be immediately compiled and displayed publicly.

After entering your data, you can see displays of the collected data of everyone's responses to date.

By entering your data, you give voluntary consent for Thyroid Patients Canada (Thyroidpatients.ca) leaders to store the data indefinitely, use it in any way they wish, and to display it publicly. Only our organization's leaders and volunteers can see the complete raw data set. The Google Form survey technology collects your IP address of your internet connection and the date and time you entered your data. We will keep that data private.  We may from time to time "clean up" the data, i.e. deleting rows of data that are incomplete, invalid, or redundant.
