Multi-Purpose Room Rental Request 
 Please fill out this form with your interest in renting one or both sides of our multi purpose room. The Rooms charge at hourly rates + amenities. Once the form has been submitted an Orem Family admin member will contact you with the agreement form. Payment must be paid 7 days after the invoice has been sent to you. If we do not receive payment during the respected timeline, we will forfeit the request.

Please note: There is a $50 cleaning fee. This fee cannot be waived.
