I hereby release the Spring Grove Youth Association, Spring Grove School District, and any of their volunteers from all liability for claims for damages which might arise because of personal injuries received in conjunction with participation in the above activity. I have read this release and fully understand its contents. I am the parent/legal guardian of the child named above and am authorized to sign this Release & Permission Slip.

I agree to accept full responsibility, financially or otherwise, for any damage my child may do to Spring Grove Area School Districts' property or others' personal property.

I understand that I am required to be in the SGYAA Cheer Open Gym Band App for all communications that include cancelations, changes, etc., and I have requested to join. *
