EPT-X & EPT ALL-STARS | Money prizes
With this form we collect needed information for the money prize payments of EPT-X Fuengirola Open, EPT All-Stars and EPT Skins matches.

Fill in the form carefully, especially check that email address is correct.

If you have multiple wins (in several events), fill in separate form for every prize.

Prizes will be paid via PayPal. We do payments once a week, most likely on Tuesdays or Wednesdays.

Prizes will be paid by a Finnish company (Natural Born Disc Golfer) and all legal issues are under Finnish taxing laws. 
- Withholding tax 15% will be reduced from the prizes of players outside Finland. Date of birth and postal address required
- From Finnish players we need personal ID number and a tax announcement (Tulorekisteri) will be done from all prizes over 100 €
