This is the registration form for the self defense seminar taking place on Saturday, May 14th from 3 to 4 pm CST. This seminar will cover a wide range of skills and will be taught by 9th Degree Black Belt, Jim Albertson. Cost is $15 per person and payment will be made at the door once you check-in. Payments can be done via cash or check. IMPORTANT NOTE - THIS SEMINAR IS FIRST COME FIRST SERVE! Therefore, the first 35 people to register can get a spot and after that we will have a waitlist.
