UPCS 6-12 Hybrid Orientation and Handbook Acceptance Form
To abide by the rules and procedures in the Health and Safety Plan, implemented due to COVID-19, including but not limited to:
Wearing a mask
Staying 6ft apart
Not sharing any items (school supplies, food, clothing, etc.)
Daily self-screening for symptoms and staying home if unwell
Covering coughs and sneezes with you inner arm
Washing your hands and sanitizing as needed
Urban Pathways believes that families are influential in students’ success. Therefore it is imperative that we work together to establish and reinforce the academic and social skills students need to be prepared for college and future careers. To that end, we are asking that each parent and child acknowledge and accept the following expectations and read and accept the Urban Pathways 6-12 Charter Schools Student Handbook policies.
To Participate in an annual orientation meeting (Parent/Guardian & Student) to review the expectations of our school
To attend all Learning Partnership Meetings (Parent/Guardian & Student) in person or via phone conference to review academic, attendance, and behavioral progress and maintain the Student’s personal education plan
To update records with accurate phone numbers, addresses, and emails so we can ensure the safety of the Student
Follow the PATH; Prepare yourself, Accept responsibility, Treat others with respect, and Help each other be safe
