Please fill out all of the requested information to register your son/daughter for the 2022 Pre-Winter Training Program (Nov 14 - Dec 23, Monday through Friday).

- Who: Any rowers enrolled in 9th grade and above (he/she can be a novice rower)
- When: 6:30-8:30pm, weekdays

- Where:
- Group A: Mon, Wed @ RHS cafeteria; Tue, Thur @ Fitness Center
- Group B: Mon, Wed @ Fitness Center; Tue, Thur @ RHS cafeteria
- Group A and B alternate on Fridays.
- Exceptions: Group A and B will meet in the Fitness Center on 11/17, 11/18, 11/21, 11/29, 12/7 and 12/13.

- The 2022 Pre-Winter Training Program is $500 (with the rebate for the Fall season included).

I, the parent/guardian of the above-named child, hereby give my full consent and approval to his/her registration and participation in Ridgewood Crew for the 2022 calendar year. RIDGEWOOD CREW COVID-19 SAFETY PROTOCOL: Core Principles - Check that you have read and agree to each item. *

1. Athletes must follow the rules and regulations set forth by the CDC and the NJ Department of Health. Training should focus on a 'get in, train, get out' approach, minimizing unnecessary contact.

2. All rowers and coaches should sanitize or wash their hands with soap for at least 20 seconds upon arrival and should follow hand-washing protocols regularly. Athletes shall carry small containers of hand sanitizer to use after touching potentially contaminated surfaces.

RIDGEWOOD CREW COVID-19 SAFETY PROTOCOL: Athlete Health Monitoring/Return to Training Guidelines - Check that you have read and agree to each item. *

1. All individuals must self-monitor for symptoms of COVID-19 daily. These symptoms include: fever, chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea. If any of these symptoms exist the person should not come to practice.

2. If an individual feels any of the symptoms mentioned in #1 during or after practice, contact a Ridgewood Crew Coach and/or Board Member and do not attend practice. The individual should reach out to his or her doctor for assistance.

3. Any individual who tests positive for COVID-19 must self-report to a Ridgewood Crew Coach and/or Board Member and complete the RHS COVID-19 Positive Case Questionnaire.

4. If an individual has had a documented case of COVID-19, they should isolate for at least 5 days from when they began experiencing symptoms before they resume participation in training on day 6 - provided they have been fever free for 24 hours without the use of fever-reducing mediations, other symptoms of COVID-19 are improving and a well-fitting mask can be worn through Day 10.
