Diploma Order Form
Please use this form to order the style of diploma you want for your high school senior. Read and complete each question carefully. The information provided here will be used exactly as provided to complete the diploma. This email address will be used for all questions and for the proof copy. Please watch for emails from transcripts@chasc.net. The quicker you respond, the quicker the process will be. Keep in mind that this process may take three weeks or longer so please order early, especially if your student will be graduating early. 

CHASC student orders are due on or before December 1 for the current school year. Diplomas are included in the graduation packet with the transcript(s) and graduation letter.

Number of Diplomas Printed on Parchment *
Enter 1 on the line below unless you want more than ONE copy of the diploma printed on parchment. If you want more than one parchment copy, enter the total number of diplomas you want printed on parchment.  Seniors must be in good standing with all forms and course information submitted on time to receive the diploma. Additional diplomas on ivory parchment are $10 each if mailed with the original copy and $25 if mailed separately. An invoice will be sent to the email address you provide. 
Name of Your Homeschool *
The homeschool name on the diploma must be the same as the homeschool name on the diploma. A name for your high school really makes the diploma more professional and complete. You can choose Homeschool, Home High School, or High School but it may not fit well depending upon the diploma style that you choose.

CHASC member students officially graduate on May 15th. You can celebrate the graduation on any day you choose but the official graduation date must be May 15th. 

Some students may graduate at the end of the summer or fall of their senior year. Generally, the date of graduation will be their final day of courses and all course information and grades must be submitted by that day. August 10th and December 10th are the recommended dates of graduation in the summer and fall respectively.

If your student is currently in the 11th grade but will be graduating early, their grade level must be changed to 12th grade. Students who graduate early essentially skip their 11th-grade year and it is essential that all students are listed in the correct grade level for high school. 

If your student is not in CHASC, you should check to see if there is an official graduation date for your association. If not, choose the date that works best for you.

Name of First Signer
This is optional if you want your name typed under the signature line followed by the title. Please type the name exactly as you wish for it to appear on the diploma. You may choose to leave this blank and have the title under the signature line. 
Title of First Signer *
You have the freedom to choose whatever title you wish. These are some commonly used titles. Obviously, it will depend upon who signs the diploma. Parents are the administrator of their homeschool so signing as administrator is perfectly legitimate. Some parents may have others who have been involved in the homeschool sign as well. "Signature of" is usually used when the name of the signer is not included.

This is optional if you want your name typed under the signature line followed by the title. Please type the name exactly as you wish for it to appear on the diploma. Leave this blank if you do not wish to have your name typed under the signature line or if there will not be a second person signing the diploma. Not all diplomas have two signers. You may also choose to leave this blank and have the title under the signature line.

You may want to change a style slightly such as use a different scripture. Be sure to include everything exactly as desired. We may or may not be able to make the changes you want. We cannot combine diplomas to make a brand new style.
Honors if Applicable
