I authorize the faith formation teachings of my child in their designated grade and on the date and hour corresponding to the Faith Formation 2022-2023 Calendar that was given to me. I understand that by signing this form my child and I accept full cooperation and participation in the sessions. My child and I will be respectful of the sessions we participate in including engaging with the assistants and catechists as well as demonstrating respect for the rules and always maintaining safe habits and behaviors. By not maintaining the code of conduct, my child and I are aware that the appropriate action will be taken and that we risk being immediately removed from the program.
