This questionnaire should only be completed by Category 1 key workers as listed in question 1. The information collected will be used to assist our planning for Monday 23rd & Tuesday 24th March 2020.

Childcare provision is open to the families of Strathaven Academy pupils, who are well and healthy and who are unable to secure personal, alternative childcare arrangements. Young people with underlying health conditions or who are currently self-isolating should NOT attend this provision.

Health and Care workers supporting COVID-19 response and associated staff

Health and Care workers supporting life threatening and critical primary and community care provision

Energy suppliers

Staff providing childcare for other category staff i.e.teachers and all other school staff including facilities staff

Emergency service workers i.e. children of police, fire and rescue, ambulance and the prison service
