Hi! I'm Dr. Jennifer Lee Johnson. I'm an Associate Professor in the Department of Community Sustainability at Michigan State University. Sherry Row invited me to Kokomo to learn from you, share what I know, and help however I can.
I put together this short survey about community health and contamination in Kokomo. I'd really appreciate it if you took a moment to complete it.
I estimate this survey will take about 5-10 min, you can take as much or as little time on it as you'd like.
Some questions ask for a simple yes or no response. Others ask for additional information in your own words.
There are no right or wrong answers to the questions below, so feel free to respond in ways that make the most sense for you.
All responses will be kept confidential.
At the end of the survey, there's an opportunity to indicate your interest in:
1) Having a longer conversation about the topic raised in this survey or anything else that's important to you. No pressure, of course.
2) Staying informed about issues of contamination in Kokomo and
