CHOIR REGISTRATION FORM
This form must be filled out for all students involved in the Farr West Choir before school program whether you are volunteering or not.  Registration closes , so hurry and fill this out before first week of class. Please read all info and fill out form and answer all the required questions.  Choir info is on the Farr West Elementary website online under the student tab, then choir tab.

Please fill out a different form for each child.   Also go through all the other info that is on the Farr West Elementary website, under the choir tab, schedules, and permissions.  If you have any questions, please email me at farrwestchoir@gmail.com.  Only fill the pic permission form out if you DO NOT want pictures and video taken...otherwise don't fill it out, just read through it.

CHOIR TSHIRT, required to have one. We are using the red one with blue microphone logo we used last year. Please check if the current one fits!  They are 6$, please pay in office.  If needing scholarship, please contact front office with questions. *
PAID CHOIR FEE?  This costs 25$ .  Please pay in Farr West Elementary Schools front office. If needing scholarship indicate below.  If you volunteer in a significant way you don't have to pay, indicate below.

Because of restrictions from the district, we aren't allowed to charge fees of more than 30$ a year.  If you already participated during Christmas 2023 and paid your registration fee, then you DO NOT need to pay for spring.  But if you didn't participate this year, you DO need to pay the 25$ for this spring  2024 season.   Sorry for the confusion.  You can always donate to the fund anyway.

HAVE YOU SIGNED UP FOR THE CHOIR  "REMIND" APP?  This is required!!!!    All responsible parties need to sign up in order to receive notifications.  You will not know what is going on if you do not sign up.  THIS IS EXTREMELY IMPORTANT AND IS REQUIRED FOR YOUR CHILD TO BE IN CHOIR.  email farrwestchoir@gmail.com with any questions.

HAVE YOU SIGNED UP FOR THE FARR WEST CHOIR FACEBOOK PAGE?  This is where you see recordings of dance choreography, lyrics and recordings to songs, info, instrument sequences to practice etc.  It also would help the children to practice any of the special parts, this will be crucial to their success.  *
HAVE YOU READ THE VIDEO AND PICTURE PERMISSION DOCUMENT UNDER THE CHOIR TAB ONLINE?  
DON'T fill this paper out unless you DON'T want your child to have pictures or videos taken of them.  But you need to READ it.  Don't fill it out unless you have concerns.  We will use any pictures and videos appropriately.  The link to the form is below.  Contact with questions FARRWESTCHOIR@GMAIL.COM

HAVE YOU READ AND PUT ALL PRACTICE AND PERFORMANCE DATES INTO ALL RESPOSIBLE PARTIES CALENDERS? these schedules are located under the choir tab on the Farr West Elementary website or below in the pic provided.  I recommend printing it off, taking a picture of the schedule, and putting into everyone's calendar. 

As a side note, for instruments, we will be doing ukulele.  The student will either need their own, or we have a couple to lend out.  We don't have enough for everyone.  It will be first come first serve.

ARE YOU, AS THE CHOIR STUDENTS GAURDIAN, INTERSTED IN VOLUNTEERING IN ANY CAPACITY? 

(no music background required) you aren't required to pay for your student fee if you volunteer in a significant way.  Also if you want your 3rd grader to participate, you volunteering can allow them to attend.  There is a meeting at the end of JANUARY for all interested volunteers.  Email with any questions. PLEASE INDICATE IF YOUR CHILD IS INTERESTED IN DOING EXTRA IN THE CHOIR.  We have special parts that are optional but require extra practices.  These include dance on Friday mornings, speaking parts, solos and instruments on Wednesday moorings indicated in the schedule, please print that off.

Your child can only tryout for 2 of the categories.  3rd graders unfortunely cannot tryout for the special parts.  There are only a few solo singing positions open.  PLEASE UNDERSTAND IF YOUR CHILD DOES NOT GET PICKED FOR THE SOLO.  

  If your child tries out for instruments, speaking, or dance...they WILL get a part.  THESE PRACTICES ARE INDICATED ON THE MAIN SCHEDULE ON THE FARR WEST ELEMENTARY WEBSITE UNDER CHOIR TAB. 

This year we will be using ukuleles for the instrument.  We don't have a lot of them...if your child wants to be apart of this, they will need to either have their own, or let us know if they need one.  It will be first come first serve. 

IF THE CHILD MISSES TRYOUTS  OR DOESN'T COME TO PRACTICE...THE CHILD MAY NOT HAVE THAT SPECIAL PART. Only sign them up if they can be to all tryouts, practices, and performances....and that they are dedicated to doing it.

We do want to accommodate any child in an organization within the school, like spirit sqaud or other leadership groups.  Sometimes they have meetings during choir practice.  Please email us with that schedule if they will miss any of the choir practices or special part practices.

Also, please type which 1 or 2 categories your child is interested in trying out for.  Instruments, solos, speaking, or dance.  This is not required if you aren't having them participate, but it is required if they will be considered and included in tryouts.  We need to know in advance who is coming so we can create the choreography and material needed for the show.  Thank you for understanding.
