  • Complete this form to request a transcript for a scholarship. You will need to fill one form per scholarship.
  • Please allow at least five school days for the processing of transcript requests. There is no immediate or rush service available.
  • After this form is submitted, your counselor will send a PDF of your transcript to your SCHOOL EMAIL ADDRESS. 
  • ***Please note that unlike with colleges, your counselor will not send your transcript directly to an institution or organization; rather, they will send you a copy of your own transcript to navigate independently. If an official transcript is needed, please indicate in form below. Student is responsible for submission of his/her/their own transcript.***
