I am aware that fighting of any kind will not be tolerated. *

I will not vandalize, damage, or violate any public or personal property on the bench, in the rink, or at any team function. *

I will not drink alcohol, smoking or use chewing tobacco, or use of illegal substances in any way as a member of my Hallock Youth Hockey team. *

I will conduct myself in a befitting manner at all facilities (ice rink, hotel, restaurant, etc.) during all team functions. *

I understand that if I do not abide by these rules, I will be subject to further disciplinary action, determined through an appearance before a Discipline Committee. *

Upon signing this document, I hereby agree to adhere to all disciplinary actions upon violation of my agreement as proposed by the Disciplinary Committee and agreed upon by the Hallock Youth Hockey Board. *
