Please complete this letter of intent form for projects for the State ESG Program. Completed letters of intent are due by 5 PM on Tuesday June 10th. Late applications will not be accepted.

Based on community need, the highest priorities for funding consideration are:
- (1) Rapid Rehousing
- (2) Emergency Shelter Case Management
- (3) Street Outreach projects.

Heartland Continuum of Care Award amount is $197,205.

Is your agency an active member of the Heartland Continuum of Care General Membership? Active membership is evidenced by participation in General Membership meetings and contributing consistently to Crisis Response task groups.

Does your organization commit to meeting to the following threshold criteria for project consideration:
(1) Funded agencies must have a Coordinated Entry Skilled Assessor on staff at all times
(2) Case managers involved in the program must participate consistently in appropriate HCoC Task Groups (for example: Coordinated Entry Case Conferencing Committee, Supportive Housing Task Group, Street Outreach, Emergency Shelter)
(3) Organization will participate in HCoC Quality Data efforts
(4) Organization will operate programs in accordance with HCoC Community Standards for Rapid Rehousing,  Street Outreach, and for ES funding assist in the completion of Emergency Shelter Community Standards
