Award Criteria:  
Outstanding Alumni Award: Given in recognition of alumni for significant contributions in professional practice, personal achievement or community service.
Rising Star Alumni Award: Given in recognition of alumni less than 10 years from their graduation date (graduates from 2010 or later), who have demonstrated significant potential, through professional accomplishments, academic honors, personal achievement and/or community involvement.

Optional Supporting Documents:
You may also include your candidate's résumé plus factual statements or any other information from other sources and any other information you feel will be relevant to the committee's decision.
