Allen Buckingham Nomination Form
The Colorado Commission on Aging will host the 31st Annual Allen Buckingham Senior Leadership Legacy Award. In order to ensure that the best possible candidates for the award are considered, the Colorado Commission on Aging is extending invitations to aging organizations, community groups, and individuals who work, volunteer, or are caretakers of Colorado seniors, to submit nominations for individuals who have excelled in one or more of the following areas:

1.  Exemplary volunteer or employment based leadership, experiences, and wisdom over time;

2.  Participation and contribution to activities that promote well-being and quality of    life for seniors and/or;

3.  Recognized accomplishments related to the field of aging.

Nominees must be a current resident of Colorado.  
Members of the Colorado Commission on Aging are not eligible to receive this award.
Please submit ONLY ONE nomination form for each nominee.  Please DO NOT submit separate letters, testimonials, or nomination forms.
