2022-2023 Faith Formation: Supplemental Information Form

Before completing this form, please be sure you have:
*VERIFIED your family information in My Own Church at https://washingtondc.parishsoftfamilysuite.com.
*REGISTERED your child(ren) for Sunday Morning Faith Formation, At Home Faith Formation, or High School Youth Ministry through ParishSoft.
*COMPLETED the online payment process.

Complete enrollment by August 15, 2022 to guarantee your child's placement in the Sunday Morning RE program. Fee: $300/child.

After August 15, 2022 children will be placed as space and catechists are available. Fee: $300/child.

New families: $200/child (regardless of enrollment date).

If you are currently unable to pay the full amount, please know that scholarships are available. Please call Anne Marie Kaufmann at 202-903-2807 or more information about financial support to enroll in our faith formation programs.

Please do not be worried by the number of sections in the form. Many are duplicate sections for families with multiple children and may be skipped if they do not apply.
