2022-23 (current) FAMILY REGISTRATION

In conjunction with completing your family's online Registration form (THIS!), the following additional items are due to the front office in order to complete your family's registration process. The items below were sent home with families previously, to begin working on:

- Saint Sebastian School's "Mutual Understanding of Support" form, signed (yellow).
- Payment method (Choice verification process complete - and a seat offered, tuition, or tithe arrangement).
- Registration fee is paid - IF your child attends through tuition or parish tithe, NOT applicable for those applying through the Choice Program. See the (green) "Payment Options" form that came home.
- Contact card (1/2 sheet, bright-pink cardstock), notifying us of your most current contact information.
- (If applicable): any updated Immunization records
- (If applicable): any updated Custodial Agreement documents
- ** "Student Technology and Network Responsibility Agreement" - coming out late Summer of 2022
- ** "Family Handbook & Policy Manual" signed acknowledgement - coming out late Summer of 2022

If applying to *3-year old, 4-year old, or 5-year old Kindergarten, please note that all children must be 3, 4, or 5 years of age, respectively, by September 1st to be eligible. We are in the process of growing our wonderful parish Preschool 3-year old program into a 3-year old Kindergarten model. More specific information will come out soon to those families who register a 3-year old child below.
