PAYMENT IN FULL: please charge my LABA studio account for the full amount of the Recital registration and costume fee(s). I understand payment will be made on April 1, 2022.

2-PAYMENT OPTION: please split my Recital registration and costume fee(s) into 2 payments. I understand the first half will be charged to my LABA account on April 1, 2022 and the remaining balance will be charged on April 15, 2022.

I understand that DRESS REHEARSAL IS MANDATORY and any student who does not attend will NOT be permitted to perform in the recital.

I understand my child may not be able to participate if they have excessive absences from class(es) and/or scheduled rehearsals.

I understand my child may not be able to participate if our studio account is not current and in good standing.

I have read the Recital information given on the Recital Page of LABA's website and agree to check the page regularly for updates and/or changes to the recital preparation and rehearsal schedules.

I understand that recital registration and costume fees are NON-REFUNDABLE should my child be removed from the performance for any of the reasons given above in the participation section.

I understand that should my child longer wish to perform in the recital, I must notify the front desk in writing. LABA will issue a refund of 50% of the recital fees up until March 15, 2022. The refund will be issued as a credit to my LABA account. Notices received after March 15 will not be eligible for a refund.

I understand that should my student need to drop out of the recital due to injury, LABA will issue a refund of 50% of the registration fee up to two weeks before the performance. The refund will be issued as a credit to my LABA account.

In the event LABA cancels the recital, I understand the registration fee will be refunded as a credit to my LABA account.

I understand if I do not want my refund in the form of a credit to my studio account, LABA will credit back the amount to my original form of payment, less any credit card fee(s).
