This form is for "Spartan Strong" buttons ONLY. The buttons do not have pictures and are $5 each. Image shown below.

ORDERING DEADLINES:
We will always be making these buttons. Once your buttons are ready, we will email you with pick up instructions. You will likely pick them up at school from Heather Fitch's classroom (#302).

Please note that ordering the buttons and paying for them are TWO SEPARATE STEPS, so reading all the information in this form will ensure that your order is placed properly.

Ordering your buttons:
1) Complete this form - Please complete all information exactly as it is requested.  Missing information will delay receipt of your buttons.
2) Please be sure to click the "SUBMIT" button at the end of the form.
*If you previously ordered a Beat Vestavia button and notified us that you would like this button as your replacement, there is no need to complete this form unless you want to order additional buttons.

Paying for your buttons:
1)  The online payment link will be displayed after you have successfully submitted the google order form. The maximum number of buttons you can pay for via the online payment link is 10.
2)  If you prefer to pay via check, you can drop off a check at the high school, attention Heather Fitch, payable to MBHS Dorians. Please note student's name and "Spartan Strong" on the check.
3) If you are paying for a larger order, you can do multiple online payments or pay one amount via check. If paying on behalf of a group/organization, please list the name of the group on the check.
4)  Buttons will not be made until payment is received.

Other information:
1) If you need more buttons, you must submit a new order form.
2)    If you have any questions or cannot get the payment link to work, please contact mbhsbragbuttons@gmail.com.
3)   You can pick up buttons for a friend but you will have to sign a sheet upon pick up.
