This form is used to collect event requests, evening exam requests, or room reservation requests that cannot be completed through virtual EMS. This form will be used to track request analytics.
Definitions
Exam request: Exams that will take place outside of regularly scheduled class time (i.e. evening exams).
Event request:
1. Any event that requires permitting (refer to event checklist for the list of
options)
2. 50 or more attendees
3. Anything requiring power beyond what the space can already accommodate
4. Any event that requires the use of staging, platforms, or other structures
5. Any event that requires us to modify or impair the fire alarm system
6. Any event that requires insurance and/or a participation liability waiver
7. Anything that requires the use of multiple spaces across campus
8. Any event that requires furniture to be moved, brought into or taken out of a space
9. Any event that requires additional A/V or Technical support
10. Events that will be serving food or alcohol
Room request:
Requests to use space on campus for things like routine meetings, study/focus groups, or smaller presentations.
*Please be advised all requests are subject to review and space availability. Classes and exams are always given priority in campus classrooms.
