Dear Parents,

It is important that we obtain an accurate list of all children qualifying for kindergarten who live in East Midvale's Boundaries for the 2020-2021 school year. If you have a child that will be five years of age on or before September 1, 2020, the child qualifies for the 2020-2021 kindergarten program. State law requires that all school districts in Utah abide by the same September 1st entrance cut-off date. Please bring, prior to, enrollment, to bring the child's immunization record and certified birth certificate (not a photocopy). We are working on an online orientation. We will get this orientation information to any that have filled out this form. This orientation will be for all students who will be attending kindergarten at East Midvale for the 2020-2021 school year.
