Dietary requirements - please be as specific as possible as we are planning the catering around everyone's needs.
Camping arrangements i.e. motorhome, campervan, large tent, small tent. Please note that Camp Plas are not able to accommodate caravans, and please be advised that some of the lanes to access the site are narrow, so please check your route before booking if you have a large vehicle. No electrical hookups are available.
Will you be sharing accommodation with any other attendees? Please note each attendee must complete a separate form!
Are there any accessibility issues you want us to be aware of?
What's your background with step dance? We'd love to know what sort of dancing you do, and how long you've been dancing.
Do you play an instrument? We'd love to know your musical interests, how long you've been playing, and if you'd be happy to play for step dancing.
What would you like to do at Camp Clog? We'd love to get a good idea of what our attendees are hoping to get out of the weekend, so we can make sure everyone gets the most out of it!
