MHS Activity Program Feedback
As part of the 9-12 Activity Review, we want to make it easier for our parents and students to complete a program evaluation. Your input is extremely important to us to continue the best possible experience for our students. It would be greatly appreciated if you would take a few minutes to complete this brief feedback form. The results of the parent and student forms will be shared with coaches in a summative format at the end of each season with the purpose of identifying areas of strength and potential for growth. A coach's assignment is not solely determined by the data collected through this process. The collection has been made anonymous.
