DMV-301 Certificate of Attendance Form Request
Beginning January 1, 2015, anyone under the age of 18 who applies for an instruction permit, driver’s license or driver authorization card must provide the DMV with proof that he or she meets the minimum Nevada school attendance requirements per NRS Chapter 392. The requirements are as follows:

-Unexcused absences should be limited to a maximum of seven (7) days per semester for our school block schedule.  
-Any day with an unexcused absence counts toward the seven (7) day limit, regardless of how many periods. A single period unexcused absence counts the same as a full day unexcused absence.
-Attendance resets every semester.

Please allow two (2) business days for processing and may be picked up from the attendance office.  For additional questions or concerns, please contact the attendance office at 702-799-8000 extension 4035.

Please note that attendance will be monitored during the semester and should the student not continue to meet the attendance requirements, their instruction permit, driver’s license or driver authorization card may be revoked.
