Thank you for your interest in Clubs! We are looking forward to a great session!

Several things to remember:
- This form must be completed by a Parent/Guardian.
- Fill out one form per student.
- Each club has a limited number of spots available, so please only submit this form for the Club(s) your student is fully committed to attend. Clubs will fill on a first-come, first-served basis. A Club Advisor will reach out to you by Tuesday, January 17, to let you know if your student has a spot in the requested club or is on the waiting list.
- Parents need to agree to the photo/image/video release posted at mytechhigh.com/photorelease.
