Pre-Arranged Absences Form

Please use this form to report any pre-arranged absences (full or half day) for your child(ren).

Each occasion for an absence must be entered separately (i.e. one submission for a vacation, another submission for a doctor's appointment, etc).

Individual forms will need to be completed for each child.

General Guidelines for Absences:

Excused: Medically-related absences; all PK absences.
Unexcused: Personal vacations and the like.
Excused with Reservation: For K only. Kindergarteners are allowed a one-time 'excused with reservation' for absences that are typically unexcused in grades 1-8 (i.e. trips)
