Parents and Guardians,

Please complete this form ONLY if you are changing your choice of instruction from the 4th Six Weeks.

The start date for this change is Monday, April 12, 2021, beginning of 5th Six Weeks.

Please note that by choosing remote instruction, you must have your own internet solution.

Per TEA guidance, parents and guardians will be able to commit to how their child(ren) will receive instruction each grading period (6 weeks).  Please be advised, if you choose remote instruction, you will not be able to change until the next grading period.  If you select face-to-face instruction and are placed on quarantine for any reason, the student(s) will transition to remote learning for the duration of the quarantine then return to face-to-face instruction.

Please be advised to participate in extracurricular activities student(s) must be enrolled and receiving face-to-face instruction.

Transfer students that were enrolled with Stockdale ISD for the 2019-2020 school year will be eligible to receive remote instruction.  All new transfers will be required to enroll in face-to-face instruction.
