AZ DANCE PROFESSIONALS
This form is being used to identify you and your dance business(es), and collect 'contact information' for our ability to communicate with all of  you on a statewide level.

NOTE: For sorting and counting purposes, complete 'ONE form' for 'each business.' This contact list will include studio owners, educators, administrators, performers, choreographers, team leaders and their dancers, dancewear stores and costume designers, and similar.

ECONOMIC IMPACT OF DANCE IN AZ for 2019
You will be one of the first to receive the survey links for the ECONOMIC IMPACT Project.

NO LONGER IN BUSINESS or JUST GETTING INTO THE DANCE BUSINESS
Still complete this form if you did not conduct business in 2019. You can answer some general questions and be counted. If you are a dancer, odds are good you did spend money on dance in 2019 and that's one of the categories we are analyzing.

Please answer the questions honestly and accurately. The goal for us is to REACH you and have you RESPOND.

The EMAIL ADDRESS COLLECTED with this survey will be your 1st choice for contact. You will be provided with the option to add a 2nd email address. List the ones you look at daily.

Thank you most sincerely, Krystyna Parafinczuk | Treasurer, Arizona Dance Coalition
