DATA PRIVACY NOTICE

The personal data collected in this Google Form will be used by the Office of Global Relations and Alumni Affairs (OGRAA) for the purpose of obtaining statistical information on the graduates of Saint Louis University primarily for feedback mechanism that can help the University in improving its programs as well as SLU's application for accreditation, re-accreditation, rankings, and international reputations purposes. This data may be shared to affiliates and Offices of the University for the purposes stated above and other assessment reports.  Any personal data collected and processed will be used only for the purpose for which it was requested.

OGRAA will ensure that protection measures are in place in compliance with the Data Privacy Act of 2012.  Furthermore, this data will be retained only for as long as it is necessary. Should there be questions or issues related to data privacy, you may contact the Data Protection Office at privacy@slu.edu.ph.

By truthfully and voluntarily accomplishing this form, I authorize the Saint Louis University - Office of Global Relations and Alumni Affairs (OGRAA) to use, process and share my personal data for the purposes described above.
