You should use this form to book a place for your child on the Gardening (Years 1 & 2) or Kew Karate (Years 3 & 4) clubs running during the second half of the summer term.

If you would like to join any of the clubs that are already running this term (Just 4 Kicks Football, Fern Hill Football or ESG Tennis England), please email us at: clubs@fernhill.rbksch.org for further information.  

Gardening and Kew Karate clubs will run for 6 weeks, starting the week of Monday 7th June 2021 and ending the week of Monday 12th July 2021. The week of Monday 19th July is a catch up week when any cancelled sessions will be made up.

Please note:

1. The clubs are for specific year groups: Gardening for Years 1 & 2; Kew Karate for Years 3 & 4.
2. Payment must be made through ParentPay at the time of booking.
3. The deadline for booking and making payment is midday (12:00 pm) on Monday 25th May 2021.

Please complete a separate form for each child you would like a book for.

Section 2 asks for information about your child, including parental and emergency contact telephone numbers and medical details. It also asks for information about who is authorised to collect your child from a club session.

Section 3 is the Extracurricular Clubs Home School Agreement. Please read it carefully and sign and date the declaration.

Section 4 is the booking form where you can select the club you would like to book.
