Eventing Ireland Self-Declaration Form - Kilguilkey International (13th - 15th August 2020)
This form must be completed by all those attending the above named event.
For clarity, each rider must complete a form while grooms/event officials must complete their own individual forms.

Please ensure that you submit this form before 17:00 on 12th August 2020. Failure to return this Self-Declaration form will result in you being turned away from the event.

We are reminding all members of Eventing Ireland who are either competing, volunteering or officiating at events that if you have travelled overseas, the recommendation is to restrict your movements for 14 days.  Please take personal responsibility for your actions to protect others.  The Covid-19 virus has not gone away and the risk of bringing back the virus to Ireland remains high.

If you have visited the Green List countries (https://www2.hse.ie/conditions/coronavirus/travel.html)  you do not need to restrict your movements, but must still declare it on your contact tracing form.

If you are returning from England, Scotland or Wales you must still restrict your movements.  The only exception is travel between the North and South of Ireland.

If you have travelled overseas and have not self-isolated, you must withdraw your entries from events if they are within the 14 day restricted movement period.  Please contact Head Office to make withdrawals.
