Age Division *

This is the GRADE your players will be in for the FALL 2019 school year. If you are a co-ed team, you must play in the boys division. Your team must enter into the age division of the OLDEST player on the team. Players can play UP an age division, but not DOWN an age division. For extenuating circumstances, please call Tom or Missy at 417-582-1107. Please keep in mind that we may call you to ask if you can play in an older division or play in a co-ed division if that helps make divisions easier to divide. We will not do this without speaking the the team representative first.

These will be your typical 100% cotton shirts. We will not be able to exchange sizes so if in doubt, pick a larger sizes. Please make sure you are double checking YOUTH or ADULT sizes. You will be given the sizes you requested.

I understand that unforeseen things happen at events like this. By signing this form, Our team acknowledges that our vehicles could get hit by a soccer ball, a canopy not secured could cause damage(although all canopies will be required to be zip tied to the fence this year). Summer Sizzler is not responsible for these damages. Park at a safe distance from the field to avoid potential damage. ALL CANOPIES will be required to be zip tied to the fence. *

As the coach or adult representative for my team, I understand that I have entered this tournament and agree that the $100 payment will be made on or prior to July 14th. If payment is not made by July 14th, I understand that the team I have registered may be dropped from the tournament. I also agree that I will read the rules of the game thoroughly before the tournament as our rules vary from other 3v3 Tournaments. *

Payment *

PLEASE READ CAREFULLY.... Once you hit SUBMIT, that will take your confirmation page and will give you a link to our Merchant for credit card processing. There is a small convenience fee to use our Merchant, but it assures you that your team has been accepted into the tournament. Please tell us who's name will the credit/debit card be in? Please keep this email handy in case you need to add/change any player names or shirt sizes. This will be the ONLY way to do any of those changes. The email will be sent to the one you have provided.

WE WILL DO OUR BEST, BUT WE CAN NOT GUARANTEE ANY REQUESTS. PLEASE DO NOT REQUEST TEAMS TO NOT PLAY AT THE SAME TIME...IF YOU'RE COACHING MULTIPLE TEAMS, PLEASE HAVE A PARENT STEP IN IF THERE IS A CONFLICT.
